Moka5 (M5) is fundamentally transforming enterprise end-user computing. Based on research conducted by Stanford University, M5 creates a highly-elastic enterprise perimeter where user data and applications are delivered as simply-managed and highly-secure workspaces to popular end-user devices. M5 enables both online and offline access and requires significantly less management overhead than legacy VDI or mobility solutions, while being significantly more secure than cloud-based file sharing, with a common platform that scales from the desktop to tablets and smartphones, Moka5 is Enterprise AnyWare™.

Benefits

+Overview
- Moka5 is committed to providing a competitive set ofbenefits to its employees and their families. This one page documents summarizes the major benefits available to employees in a concise format. As you come on board we will be providing you more detailed information about your benefits. If you have any questions please ask your hiring manager.

+Premiums
- For medical and dental, Moka5 pays approximately 80% of employee premiums and 50% of premiums for family members. For vision and dental coverage, a minimal amount is paid by the employee. For the life and disability benefits, Moka5 pays 100% of the premiums.

+Medical
- Five choices of plans are offered: Anthem Blue Cross PPO with a $10 co-pay, HMO with a $10 co-pay, Lumenos HSA1500, Lumenos HSA2500 (high deductible accounts), and a Kaiser HMO with a $15 co-pay.

+Dental
- MetLife Dental plan is offered. This program pays for 100% of preventative treatments and tiered reimbursements for other services.

+Vision
- Vision Service Plan is offered. Eye exams every twelve months for in-network providers are a $10 co-payment and lens with a $25 co-pay. Frames are paid for every two years up to $120 together with a discount.

+Life Insurance
- Through UNUM, basic life, accidental death and dismemberment (AD&D;) coverage is provided. The company pays 100% of the premiums. Coverage is for the employee’s salary up to a maximum of $250,000.

+Short Term Disability
- Through UNUM, short term disability (STD) coverage is provided. The company pays 100% of the premiums. Coverage starts after 7 days of sickness/disability and continues for a maximum of 12 weeks at 2/3 your pre-disability earnings up to a maximum of $2,309/week.

+Long Term Disability
- Through UNUM, long term disability (LTD) coverage is provided. The company pays 100% of the premiums. Coverage starts after 90 days of sickness/disability. Coverage is at 2/3 of your pre-disability earnings up to a maximum of $10,000/month.

+Employee Assistance
- Confidential counseling on a variety of issues such as relationship, marriage, family, life change, stress, alcohol and drug dependency and more are available through the employee assistance program offered by UNUM. The company pays 100% of the premiums.

+401K
- The company offers both a traditional 401K and a Roth 401K through Fidelity Investments. Employee funds can be directly invested into a variety of mutual fund choices.

+FSA/HSA
- FSA – Through ADP, employees are able to deduct money on a pre-tax basis to pay for medical expenses and dependent Care.
  
  HSA – For the Lumenos HSA1500 and Lumenos HSA2500 options, the employee must set up the account with his/her banking institution and Moka5 will deduct money on a pre-tax basis for use with the Limited Purpose Flexible Spending Account (LPFSA).

- A workers compensation insurance policy is in place for any on the job injuries.
